| Bargarh Integrated Community Health Project: BICHP |
The main aim of the project is to reduce the leprosy prevalence and negative consequences (Disabling consequences) of the disease and to sustain the leprosy services within General Health Care System by utilizing community based participatory approaches.

| Project Success and Major Achievement |
During the 5 years 9 months (Jan.2002 to Sept.2007) project period 4200 patients were treated and released from treatment at Govt. Health Institutions facilitated by the project. Out of 1550 persons with disabilities, 652 became self reliant, 208 are being taken care by GHC staff, 375 by partners/HRC members and the rest 615 by project staff. During the project period 694 ulcers healed, 176 new impairments prevented and 400 persons with disabilities have undergone corrective surgery. With SER interventions188 clients and 367 family members have been incorporated in existing self help groups, 72 patients have been benefited through LEPRA revolving loan, of which 40 clients life style has improved well. 10 clients through philanthropic support and 580 patients benefited through various Govt. welfare schemes. In community mobilization, 129 active partners identified, 30 Micro Plans prepared, 31 Village Health Committees and 24 Health Resource Centres are established by the community through active facilitation of the project. The 20 HRCs proactively engaged in rendering support to people on health awareness and services must be sustained in the long run. 112 POD kits were provided to Health staff, HRC, AWWs, Partners and Traditional healers and utilized for clients.
One of the key objectives of the project was to work hand in hand with the Government and the project participated and contributed to the success of MLEC, LEC, SAPEL, BLAC-I, BLAC-II (special leprosy elimination campaigns). In the process the project could bring success in reduction of PR from 24 to 1.9 NCDR from 35.1 to 2.8, DR from 8.5 to 3.7 during the five years 9 months of project implementation. The project also participated and contributed to the successful implementation of other Govt. health programs like FHAC (Family Health Awareness campaign), Pulse Polio, Mega Health Camp, Reproductive Child Health Camps, Leprosy/Malaria/Filaria Samadhan Sibir and observation of important health days.
The project derived much response from the community in aspects of ulcer dressing and follow up, self care teaching, referral of suspects to GHS, referral of clients to Govt. for welfare assistance there by, sustaining the effort on community ownership of the programme. The 129 active partners who had the real motivation continued to work with the project till today. The project took measures to restructure and strengthen the HRCs afresh where representative from all age and caste group were involved to look after the functioning. The strength of the project is the well equipped infrastructure in the field of leprosy, strong networking and collaboration with Dist Authority; local Community Based Organizations (CBOs) and partners. |

The IEC van with local specific IEC materials has been an asset to district health administration to create awareness of people on major health diseases including leprosy. The community structure (HRC) and partners’ continued involvement in the project activities can also be considered as strength for future project implementation. The self reporting rate increased from 48.5 % to 100% over the 5 years project period by conducting contextual IEC programmes in the project area. All the patients took regular treatment and cured in time. The cases under treatment are taking treatment regularly. Increased the level of awareness and patient self-care of their affected limbs and community involvement in project activities and community performed many awareness activities on health and development. |

| During the 5 years 9 months the Project conducted series of trainings, sensitizations and workshop to GHC Staff, ICDS Staff, HRCs, VHCs, SHGs, NGOs, CBOs, PMPs and different community groups, clients and their family members on different topics of leprosy, Malaria, community mobilization and health interventions. As result improved the knowledge and skills of GHC staff and HRC members, they involved in MDT services, increased involvement in POD Services, community groups (HRC) taken ownership and a link has been established between GHC System and Community towards patient care and services. |

Community also involved in health awareness, service oriented health camps, conducting disability care clinics, malaria awareness and treatment camps. As a result there is a gross change in important indicators of leprosy and malaria. In addition to this the project also conducted different training for staff on development, by this the community and GHC staff involvement increased in health interventions towards Programme and project success as well as for better patient care.
Lepra recognized as active coordinating member of dist health administration as a Joint secretary of Dist TB and HIV association, member of DRHM Planning and Monitoring committee, Member of Zilla Swasthya Samiti (ZSS) and Vice President of District Vigilance Committee etc.
In Malaria Programme build the capacity of staff on Malaria, conducted Intensive IEC activities and Expansion of out reach activities, Involving GHC staff, ICDS Staff and Community) mainly focused on targeted community mobilization villages. By these 2512 persons examined on the camp site of Six Malaria Samadhan Sibirs, 419 persons reported voluntary with Malaria symptoms, 362 Blood Slides collected and examined on the spot, presumptive treatment provide for 362 persons and Radical treatment given to 59 positive cases. All the treated cases cured from Malaria and only 4 cases having complications referred to specialized centers.14 villages regularly continuing Malaria Sanitation, once monthly (Total Village cleaning) with the active community/HRC/SHG involvement. In the targeted area155 none functioning DDCs, 50 FTDs and 50 MLVs functioning actively and providing malaria treatment to sufferers. People using Insecticide treated Mosquito Bed Nets (ITBNs) regularly and cultivating Larvivorous fish in the ponds having stagnant water. Increased community involvement in Malaria preventive and curative practices and voluntary reporting of Malaria suspects increased at health institutions.
By all these the Annual Parasite Incidence reduced form 5.4 to 3.1 and people knowledgeable about Malaria cause, signs/symptoms and treatment facilities and practicing both preventive and curative measures in these limited target villages. Project point of view the Dist health and administration pleased with the Project Malaria efforts and results and requested Lepra to apply to EDPT and IVC schemes of Public Private Partnership under NVBDCP.
As per the request of the Dist health and administration submitted a proposal to cover 5 Blocks for specific interventions (Bargarh, Attabira, Bheden, Ambabhona and Bhatli) of Bargarh Sub division for Malaria under Public Private Partnership of National Vector Born Disease Control Programme in March -2007 for 3 years project with proposed budget of Rs.23, 59604/- (Average Rs. 8 lakhs per annum). The proposal mainly focused on 3 different Schemes, namely
- Provision of outreach services – DDC and FTD
- Provision of Microscopy and treatment services (4 Microscopy Centers)
- Promotion of insecticide treated bed nets, insecticide treatment of community owned bed nets.
The ZSS Bargarh and Dist NGO committee approved the same and forwarded to State Govt for final approval. |

| Achievement against Major Activities – 2007 |
Direct Service Delivery in Prevention & Care of leprosy disability |
- Out of 120 targeted clients of the year 89 become self reliant in taking care of their affected limbs and socio economically self sustained
- The Project could provide ulcer care and self care learning’s to 251 ulcer clients through 63 nos of disability care clinics
- Similarly 35 no. of NFI cases were treated, 23 recovered fully and the remaining 12 are under regular supervision and treatment.
- The Project could also heal 67 no of Simple ulcers and could convert 17 nos of complicated Ulcers into Simple one.
- The Project has provided 73 pairs of G-I and 343 Pairs of GII and 52 adaptive devices to needy clients.
- Nearly 42 nos of clients having complications, reaction and neuritis have been provided with timely service and medicines in 137 Episodes during field visits, PHCs and Physio unit (from project and Govt) thereby preventing of further worsening of the limbs.
- 64 nos of pre operative cases were screened, amongst them 38 cases are duly motivated and referred for surgery whereby all of them have got their limbs corrected from project area and 53 cases operated from non project area (Padampur subdivision and existent districts of western Orissa).
- Apart from the same 231 cases are given post operative Physio management at the project and 214 have responded well with desired results of surgery.
- The Project has conducted 18 nos of DCCs with active support from members of HRCs And GHC Staff.
- Conducted 74 Clinics at project Physio unit (Referral Centre) during the quarter. The project MO/PT provided the required services to 804 needy clients with required number to times (Reaction -151, Neuritis - 86, Ulcer care - 199, Footwear - 69, preoperative management - 44 cases post operative Physio management to 141 clients and plaster application – 52 clients).

Information, Education & Communication in Leprosy and Malaria |
- With the active involvement of the Project as far as Malaria awareness in concerned 2 GPs, about 17 no of DDC/FTD are functioning well. 10 target villagers continuing the sanitation drive in their respective villages.
- Three Leprosy/ Malaria Awareness and Treatment camps conducted in 2 target GPs. 252 persons treated at the camp site, of which 121 positive cases.
- 1000 people visited these camps and awared about preventive and curative measures f Malaria and Leprosy.
- Community actively participated with the support from the Project in observation of important health days like wise Anti Leprosy Week, World TB day, World Health Day, World Red Cross day, Anti Malaria Month, Republic day and Independence Day in 14 Target Villages and all the HRC villages.
- Through IEC a total of 64138 students, women and community members were made aware through Various IEC activities i.e. Group meetings, FGD, School Education and drama/palla in prominent Colleges, Exhibition, Film show, observance of Anti Leprosy Week, World Health day, Dhanuyatra And Anti Malaria Month for disseminating messages on Leprosy, Malaria and other allied diseases. This also resulted in instant awareness and voluntary reporting of 198 suspects on the spot during Programmes and many reported at PHCs, as a result of which 207 new leprosy cases (109PB and 98 MB) Have been confirmed early and are under medication at Govt.Health Institutions.

| Capacity Building to GHC Staff, ICDS Staff, ASHA and community structures |
- As far as Training and Capacity Building are concerned the project conducted 30 Trainings during the quarter, 6 for ASHA, 12 for HRC/VHC and Partners, 2 for PMPs on Leprosy Disability, Complication Management and Malaria, 1 Press/Media Workshop, 3PRI Sensitizations, 4 workshops for Stakeholders on SER, 2Trisector Interface workshops, wherein 942 persons participated (M-504, F-398).
- By the impact of the training programmes the participants referred 3 TB suspects, 17 Malaria Suspects, 3 leprosy suspects to near by health institution. The participants Provided disability care and services to 20 persons, referred 2 cases for RCS, 5 clients got disability pension, 15 clients names processed to Govt for economic benefit through different Govt Schemes, 1 client mainstreamed into community 18 clients needs assessed by the participants.( This information on training impact collected from the participants after 3 months of completation of the training in a designed feed back format of trainings).

Socio Economic Rehabilitation to Cured Leprosy Persons |
- The Project in this quarter has supported 55 nos of clients economically by able linking them with Government schemes and has also incorporated 4 clients and 15 family members of clients in Self Help Groups (SHGs).
- 3 No. of clients with due support and encouragements from the project has got elected in the Panchayat Elections and 4 No, of clients have been selected as e ASHA under NRHM.
- 84 Clients Social problems solved with necessary counselling on self/social stigma reduction. The clients were accepted by community and presently they are living in their respective villages without any stigma and discrimination.
- The project has supported 42 clients with various Govt schemes for housing, subsidized food grains etc.

| Community Mobilization/Involvement in project interventions |
- The Project has established linkages with 20 Health resource centres (GP level-Planned) and 30 health committees which have resulted in mainstreaming local health agenda into their activities.
- Celebration of Nuakhai Bhetghat (Nuakhai Festival) of western Orissa at Jamurda (Bargaon Kushthashram) on 20th September 07 in collaboration with Bargarh District Press and Media Association with an aim to mainstream the leprosy affected living in colonies (Social Integration).
- HRC Rusuda conducted one child health camp and provided necessary child health care services and medicines to 150 needy children; The Project facilitated the camp the collaboration of NRHM. The sample Pediatric Medicines provided freely by the Child specialists of Bargarh town.
- Bhainatora HRC conducted one Filaria awareness and treatment and provided necessary treatment and self care education to 72 people affected with Filaria. Project provided the technical support and Benacideforte tablets freely supplied by Dist Druggist and Chemist Association.

| Networking & Advocacy with Govt |
- Continuous liaison with Govt and other agencies working for social and economic development of deprived classes have been maintained in these 9 months. The CDMO, ADMO (PH) and DPM, NRHM Bargarh actively participated and presented their views and suggestions for a comprehensive Action Plan – 2008 Preparations.
- The project in collaboration with District Water and Sanitation Mission has conducted sanitation drives to combat in 8 villages the project undertook motivational drive to use medicated bed nets. Monapada HRC actively involved in total success of sanitation in the GP with the active involvement of the community, which is expected to get the Nirmal Gram award
- On the eve of Anti Leprosy Week Observation and its valedictory function the DM & Collector attend the function at BICHP project office with dist health authorities and expressed his appreciation and also requested LEPRA to extend the leprosy activities to Padampur Subdivision writtenly and orally.
- On the eve of World Health Day, Bargarh MLA attended the Programmes at DPEP Conference hall with dist health authorities and expressed his appreciation on project interventions.
- Some of the results based important networking initiatives are the Collaborative implementation of Health Camp under Red Cross Society at Ainlapali GP covering 5 villages, a prominent and need based health service area of Bheden CHC About 382 needy patients were benefited of which 265 Adults and 117children. Like wise Project provided technical support to SANJOG Organization on Conduction of a Health Camp at Talsrigida of Katapali PHC, mainly on awareness generation about the camp and the available treatment facilities, 280 needy patients benefited by this camp with necessary treatment and medicines. Conducted Reproductive Child Health Camps under NRHM in three prominent need based health service areas at Lakhanpur of Bhukta PHC, Patrapali of Attabira CHC, Dathukipali of Bheden CHC covering 15 sub centers of 35 villages. About 1556 patients were benefited of 608 mothers and 549 children.
- By good rapport with Dist health and administration, as per their invitation the project senior staffs attended the Malaria Task Force Meeting on observation of Anti Malaria Month for implementation of different activities on malaria prevention, Gram sate programme at DRDA Conference hall on Malaria, Positive Deviance programme under DSWO for child growth, nutritious food and education and also attend as a member of Dist Selection Committee for PPP – PHC for selection of NGOs to take up primary health centres.
- After a series of advocacy initiatives the DPM and Dist Accounts Manager, NRHM, Bargarh and BPOs of Bhukta PHC/Attabira CHC visited 3 HRCs and 3VHCs to see their involvement in health interventions in September-07 and appreciated the activities performed by these community structures, the community support and participation to HRC and HRC activities and the monitoring tool and its update. They assured to recognize the HRC/VHCs as NRHM functionaries and provide the untied fund and health schemes to these centers.

- In Public Private Partnership, recently the Dist Provided 8225 Insecticide Treated Mosquito nets to project to distribute the same to the BPL and APL families as per NVBDCP norms. The responsibility given to 2 HRCs of the respective area (Rusuda and Banipali), by this initiative the HRC members will be economically benefited and actively involved in the programme.
- Formed a SHG consisting of 5 Disability clients and linked them SGSY Scheme.
- By liasoning with PHC and PRI members emergent problem of a client Mr. Patra Haripal has been solved.

- Health message dissemination on Leprosy and Malaria through local spiritual discourse- Bhagabat Sabha Palla, Daskatia and kirtan mandali etc. Which are attracting the people and increasing the interest of the people to know more about preventive and curative measures of different diseases?
- Involvement of HRC members intensively to make Monapada GP as Nirmal Gram under District Water and Sanitation Mission.
- Micro Plan displayed at different HRCs with wall painting of entire village social map with the identified problems with time frame/responsibility to solve the problems.
- Monitoring tool to monitor important health indicators of the village and GP displayed at VHC and HRC and they are using properly.

| Considering the high prevalence of leprosy and high disability load the project will continue its leprosy services in the year 2008. Apart from this, as there are some high malaria endemic pockets in the district, the project has been offered to work for malaria control in the district in a partnership mode with General Health System under Public-Private Partnership (PPP) as per the guidelines of National Vector-Borne Disease Control Program (NVBDCP). |
