Improving quality of care of people living with HIV/AIDS in an integrated approach |

Objectives:
- Keeping them comfortable and prevention of opportunistic infections that can make them feel worse.
- Assisting them in coping with the continuing illness they experience.
- Keeping them within the community and family groups as long as possible. This can provide them with a better quality of life at a time when they are too ill to enjoy or understand what is going on.
- Helping them and their families prepare for coping with life, after HIV. This may include proper care and nutrition, tending to relationships in the family or the community, arranging for the transfer of responsibilities and helping them to be as independent as possible.
- AIDS patient requires more than a medicine or hospital to cope with his physical stress, mental agony, social, psychological and spiritual conditions. S/He needs holistic care which should heave more emphasis on nursing care like that of physical care to keep the person clean and dry, to prevent skin problems, stiffness of joints and relieve pain.
- Crating an enabling environment among them for sustainability through economic activities and entertainment through recreation.

| Inward Patient Care (diagnosis & treatment, nutrition, Recreational) |
Inward patient care services started from 9th April of this year. Till September 2007, 111 PLHA have been admitted in the center so far & got the facilities of the center. Out of 111 admitted cases, 52 cases have taken readmission with other opportunistic complications. During their short stay of 15 days they have cared in following way
- They have given nutritional support as per Centers menu chart
- They have given Psychological support including counseling, Yoga & spiritual discourse
- During the stay of clients they have provided various recreational facilities like watching TV, Playing indoor games, gardening& sharing jokes, songs, stories etc.
- They have provided counseling on home based care & for prepare them to Cope with present conditions.

OPD Service:
During this 8 months project intervention there are 61 PLHA attended OPD of CCC for Counseling & treatment purpose. At the time of OPD visit they have provided counseling on Care & support program of NACO like ART and other things. Simultaneously they have provided medicines of mild opportunistic infections & STD complications as per advice of Doctor.

Yoga Class:
Regularly Yoga Instructor of the project is conducting the Yoga class for in ward clients. With them the clinical staffs are also attending the same. There are 25 in ward clients have attended Yoga classes fully & practicing the same at there home.
In between the project had conducted 7 days Yoga Session for all staff of CCC. The health workers are used to facilitation of Yoga in doorsteps of client. Simultaneously Clinical staffs, especially Nurses and Janitors are taking Yoga Class to inward clients in absence of YOGA Instructor.

Referral Services:
During this period, the Center has done referrals in following way
- Referral for HIV testing – 13
- Referral to ART Center for CD4 count & accessing ARV drugs. - 31 clients
- Referral to other Health institutions for better treatment of patients- 15 clients.

Community Mobilization Meeting:
Project has deployed 5 health workers for field level services. During this period they have conducted 59 Community Mobilization meetings so far and following topics they have covered.

- Misconception on HIV/AIDS
- Social Stigma & Discrimination

Client Follow-up at Door Steps:
Regularly the field workers are doing follow-up with clients at their door steps. There are 178 people living with HIV/AIDS are registered in Community Care Center. Field workers have managed to follow-up all 178 clients in their doorsteps. In their follow-up they have covered followings:
- Home Based Care (Nutrition, universal precautions etc.)
- Yoga
- Accessing the need of clients
- Linkages as per required of Clients
- Information on Care & support program
- Counseling to Clients who are taking ARV drugs
- Referral to CCC who have complications
- Referral to other Health institutions as per required.

Counseling
The Center has provided counseling to clients as well as his/her family members at field level and at center also. There are 105 clients have counseled in both field & center level. Simultaneously 112 family members have also counseled by field workers and clinical staffs so far. |
Networking (Inter Scrotal Linkages)
In continuation of networking with different stakeholders such as govt. (Health and general administration), NGOs and industries, the project strengthened linkages with VCCTCs of Koraput, Jeypore, Malkangiri and Nabarangpur. Project also initiated the process of establishing linkages with VCCTC of Rayagada district. Through the process of networking and linkages referrals from districts other than Koraput has also been increased. The project has initiated the process of facilitating positive networks in Malkangiri & Nawarangpur district. The project also refers cases to treat Tuberculosis and STD to respective designated microscopic centers and STD clinics respectively. |
