Operational areas of HIV Division Projects
 

Introduction:


The operations of HIV interventions in AP of LEPRA Society in the beginning of the year 2005 were under AP Regional office. During the latter part of the year because of the additional major new initiatives approved by the donors there was an emerging need to establish a new HIV division, which was created from 1st November 2005 imbibing the LEPRA Society’s Mission Statement in word and spirit. This could happen as part of the re-structurisation process of the organization, which was approved by the Management Committee in October 2005.


At the beginning of the year 2005 the HIV interventions which were operational are the Partnership for Sexual Health - Truckers project; Frontiers Prevention Project and Andhra Pradesh Counselors Project . The new initiatives developed during the year 2005 are Cheyutha project supported by LEPRA Society; Jyoti project supported by CDC GAP-India, APSACS -GFATM supported PPTCTC plus, TB-HIV Coordination & ART role out project in AP. Taking into account these new developments the HIV division was created with an agreed organogram (please refer to Annexure 1) to make it operational with an understanding to focus on the direct HIV projects in AP and to provide the technical support in developing and implementing new HIV projects. Presently 462 staff is recruited in the division.


HIV Division - SPANDANA is a decentralized office responsible for:
Management of the Operations in the direct HIV projects namely Jyoti Project, Young People’s Initiative, PPTCTC Plus and SPANDANA.
Establish the Operational / Technical / Administrative systems for implementing and monitoring the HIV activities in LEPRA Society
Liaison with the State & Central Governments and also with donor agencies.
New initiatives, monitoring and evaluations are being planned / conducted in coordination with the Regional Offices and Head Office.

Progress in 2006:

In April 2006, SPANDANA handed over the Projects namely Partnership for Sexual Health – Truckers (PSH-T), CHEYUTHA and other new initiatives with Alliance (IHAA) to Regional Office – AP by for line managing them directly. The budget for HIV Division - SPANDANA was approved during first forecast of 2006.

The New Initiatives undertaken by SPANDANA from Jan 2006 till date in coordination with Regional Offices, Head Office and Dept of New Initiatives.

Action Plan for 2007:

The HIV division is consolidating the progress made in the year 2006. The new plans for the year 2007, keeping in view the NACP III and HIV strategy of LEPRA Society, will focus on the following:
1.
Supervise on site training, practical, function specific and workplace specific training and promote checklists for supervision for all HIV based projects
2.
Increase the frequency of review meetings for interdepartmental growth.
3.
Increase manpower in the projects of the HIV division and also in the HR and IT departments of HIV Division.
4.
Upgrade the divisional & staff information system at district level.
5. Promote positive people’s groups network at district level.
6. Linkup district level networks.
7. Promote inter-sectoral networking and awareness about HIV projects.
8. Develop a training institute with support from suitable donors
9. The monitoring plan /Strategy being planned, for the projects for the year would focus on monitoring to enhance the quality of services provided under the projects.
10. To develop a good comprehensive web based reporting & monitoring for the APCP
11. Documenting, Disseminating and Publishing the best practices and challenges in the field interventions
12. Organizing capacity building training for the staff of HIV Division

There are plans to undertake operational research during 2006-07 in various thematic areas such as
Discordant couples
Stress & burn out among counselors
Client satisfaction & climate determinacy studies.
Psycho-somatic conditions of widow spouses
Sexual determinant factors among positive people
Life style practices of positive couples in rural and tribal areas
Effective communication & disease management in community health
Role of peers in PLWHA management
   
 

New initiatives



Mobile VCTC van for Hyderabad – On formal request from the District collector of Hyderabad, LEPRA Society, with support from APSACS and the District Collector office, is launching a Mobile VCTC for Hyderabad. It would serve the unreached population providing counseling and testing services, referral services and treatment for STIs.

Mr. Ravi (name changed) is a forty five year old sand seller. He also owns a hotel at his native village Yeshwanthpur. He looked after the sand business and, his wife used to take care of the village hotel. Both husband and wife had extra marital affairs. Nine years back his wife expired. A year after his wife’s death he married again.  From his second marriage he has a six year old boy. 

He had a prolonged illness, and was referred to the nearby VCTC.  After pretest counseling he was sent for HIV Test. Results revealed him to be positive.  The counselor motivated his second wife and the 6 year old child to go for HIV testing. Test revealed both to be positive.  In post test counseling the counselor counseled the couple and explained about taking care of their health, safe sex practices and importance of education for their son. Now both are taking care of their health, taking nutritious food at home, availing medical support from LODI SOCIALSERVICE SOCIETY.                   

Mr. Ravi’s son from his first wife is 27 years old and is working in Mumbai. He knows about his parents HIV positive status and has accepted it, weekly once, he calls them to enquire about their health condition. In this case the second wife also had an extra marital affair with a young tailor. The counselor counseled this person and motivated him to take the test, the results revealed him to be negative. Now all these persons come to the VCTC for follow up every fifteen days.  Their health condition is good at present and they have changed their lifestyles due to effective counseling and regular follow up.
 

Important visitors to the project

 

A team from Karnataka State AIDS Control Society visited the Division to learn about the Andhra Pradesh Counselor’s Project. They were provided with information on the geographical extent, the management structure, functioning and monitoring tools of Andhra Pradesh Counselors Project.

Training of Trainers on Prevention of Parent to Child Transmission (PPTCT) - The Training of Trainers on PPTCT was organized for obstetricians/pediatricians/gynecologist. 40 Medical professionals from teaching hospitals from eight districts attended the 3 day workshop. The TOT was inaugurated by Shri G. Asok Kumar, Project Director APSACS, Dr. Kalidas, Assistant Project Director, APSACS, Dr. Tripti Pensi, UNICEF representative, Dr. Shalini Shah, UNICEF, Wg. Cdr. Bose, Director HR, Admn. & Finance, LEPRA Society and Dr. Satyavathi. The objective of the training program was to equip the participants with skills and knowledge to motivate and train their juniors and other hospital staff in doing positive deliveries, safely and the necessary medical measures required to ensure that the baby born is non reactive. The TOT at State level was followed by Zonal wise TOTs for medical officers of Area Hospitals, Community Health Centres. The aim is to sensitize the doctors so as to motivate them to do positive deliveries as well as encourage other hospital staff to treat HIV positive patients without stigma or discrimination. The modules were similar to the ones used in TOT at State level.
 
 

A ‘Well Baby’ show was organized by a project called Prevention of Parent to Child Transmission plus project to showcase the healthy babies born to positive mothers. One show was organized in Niloufer hospital. Sixty babies participated in the show with their parents. The babies included both reactive and non-reactive babies. Some healthy babies were given prizes. The program was an encouragement for the positive mothers and an inspiration for others to come for institutional deliveries get tested for HIV and ensure the child born is healthy.

 
Life Skill Education Workshop: A residential workshop on Life Skill Education was organized from 8th to 10th August 2007 by the Young people Initiative (YPI) project. 4 participants from each partner NGO of YPI attended the program. The workshop was organized to build the capacities of the four NGO partner's staff in Life skills which they will impart during their ongoing field activities. A team of three resource persons from WINS facilitated the program.
 
 
 
Bold Doctors Campaign - A ‘Bold Doctors campaign’ was initiated by the Prevention of Parent to Child Transmission plus project. This campaign aims at sensitizing medical professionals and motivating them to undertake positive deliveries. The inaugural program was attended by Mr. G. Asok Kumar, Project Director, APSACS. 53 bold doctors were felicitated in order to inspire the other participants to do positive deliveries. The campaign will in the form of sensitization meetings which will be organized in different venues in Hyderabad and Secunderabad, medical personnel from both private and government hospitals will be motivated to participate in the program.
